Desk rejection is the most avoidable failure in pharma publishing. A paper is rejected by the editor before it ever reaches peer review, often within 48 hours, and the team loses weeks before they even know why. In our experience, the majority of desk rejections are not scientific judgements. They are process failures.
1. The journal was never the right fit
When journal selection is driven by impact factor alone, scope mismatch is almost guaranteed. Editors desk-reject papers that fall outside their aims and scope, regardless of quality. Journal fit should be engineered before writing begins, using acceptance rates, scope analysis and desk-rejection risk, not just impact factor.
2. Reporting guidelines were retrofitted
CONSORT, PRISMA and STROBE cannot be bolted on at the end. When a manuscript is written without the relevant checklist in mind, the structure itself is wrong, and editors notice immediately. Embedding the guideline from the first draft is the single highest-leverage change a team can make.
3. Formatting and submission readiness were an afterthought
Reference style errors, missing structured abstracts, figures that do not meet specification, these technical defects trigger desk rejection even when the science is strong. A dedicated technical QC layer catches them before they reach the portal.
What to do instead
Treat pre-submission as an engineering problem. At Ichelon, the Pre-Submission Scientific Risk Engineering Model and the 4-layer Publication Readiness System™ are designed precisely to remove desk-rejection risk before submission, journal fit first, guidelines embedded, technical readiness governed.
